John E. Kostic Memorial Foundation Juvenile Diabetes Scholarship

The John E. Kostic Memorial Foundation Juvenile Diabetes Scholarship provides monetary awards to students with type 1 diabetes who are pursuing higher education. It is intended for high school seniors planning to attend an accredited four-year university, college, technical or trade school. Winners are notified by email and checks are distributed directly to the recipient's school after verification of acceptance and registration. The number of recipients and award amounts vary each year.

High school seniors with type 1 diabetes who are New Jersey residents and who plan to attend an accredited four-year university, college, technical or trade school; must participate in community or extracurricular activities or sports, demonstrate good academic performance, and have an endorsement from a physician or Certified Diabetes Educator (HbA1c is not a criterion).

Complete application and submit proof of citizenship (state ID or US passport), transcript, list of extracurricular activities, a letter of recommendation from a teacher/coach/counselor, an endorsement from a physician or CDE, and a 500-word essay on the importance of the prospective major.
